Instructions for Scheduling Your Non-Member or Urgent Care Appointment

  1. Complete the Form Below: Fill out the required fields to provide us with the information we need to prepare for your appointment.
  2. Submit Your Payment: Use our secure payment system to pay the $200 one-time telehealth or $275 in-person consultation fee.
  3. Access the Scheduling Page: After your payment is processed, you’ll be redirected to a private scheduling page to book your appointment.
  4. Fill out Medical Intake Forms: These forms allow us to gather important information about your health history. These and current concerns will help to better prepare for your consultation. Please ensure all information is accurate and complete before submitting. This step is required to finalize your appointment.
  5. Finalize Your Appointment: Once booked, our provider will review your information. They will then confirm your aapointment and meet with you during your chosen time slot.
