Our Membership plan gives you direct access to personalized care from a dedicated provider, both in-person and via Telehealth. We also offer non-member care for those not ready to join a membership. Our Membership Plans are paid for Monthly. Enrolling in a monthly subscription continues until you choose to cancel. We only ask for a 30-day notice to cancel, and regular charges will continue during that 30-day period.
* Our pricing is reviewed periodically and may be adjusted for new memberships. However, active members will retain their current rates for the duration of their membership term.
** In-person visits are included within a 20-mile service radius. Travel beyond this distance may incur additional fees, which will be communicated prior to the visit.
